David was born March 10, 1963 and Millersburg, Ohio to Bobby Monroe Cook and Dana O. (Angle) Cook Hawkins.
David loved "driving truck" and spent a lot of that time with his brother. He had a big heart, many friends and a here-and-now attitude. If you met him you didn't forget him. The family will miss his giving heart and carefree personality.
He is survived by his mother, Dana (Lloyd) Hawkins of Killbuck, Ohio; sisters, Sandra Whitley and Tina (Mike) Galford both of Killbuck; and a brother, Arthur (Lori) Cook of Walkerton, Indiana.
David did not have children of his own but loved his nieces and nephews dearly, Josh, Artie, Adam and Sherry Cook all of Indiana, Jerimie and Tracy Ellis, Angela Chaney, Ashley Watts, Manda Jackson, and Devin Williams. There are also several great nieces and nephews and extended family. He is also survived by Jesse Baker who has been a close, lifelong friend since kindergarten.
David is now united with his grandparents, father and friends who have gone before.
Fly high my brother, there is no more pain. Run and do not stumble, there are no obstacles in your way now.
"Thank you Momma for praying for me."
